RE: Request for Information
It is important to Washington University that key contractors/vendors are properly positioned to be able to sustain their business relationship with the University. Financial health, safety practices and appropriate levels of insurance coverage are important indicators of a company’s ability to fulfill contractual obligations. With that in mind, we ask you to provide us with the information requested within this application, including required attachments. This application is comprised of three sections which need to be completed in their entirety. There are sections for each of the following areas:
- Financial
- Environmental Health and Safety
- Insurance
We will review the information you provide as we assess your company’s ability to perform certain work for Washington University. Your responses may lead us to request additional information, and we may ask for this information on a periodic basis.
Note that your responses to all requested information are to be based on your entire company, not solely on business you do with Washington University.
When the application is completed in its entirety, please submit it via PMWeb for consideration.
